Fix PHPUnit tests

Link.php: SIP Links get automatically parameter '__dbIndexData'
Report.php: can select DB via 'dbIndex' directive (global)
AbstractBuildForm.php: dbIndexData now based on setting in Form (before on config.qfq.ini)
formEditor.sql: FormEditor update on Form 'form' and 'formElements'

Reformat manual for config.qfq.ini. Copy config.qfq.example.ini to MANUAL.rst. Migrate config defaults from setIfNotSet() to array_merge().

Feature 5425 / thumbnail: rewrite - secure thumbnails are now rendered on first access, not when 'AS _thumbnail' is called.

New FE.parameter 'inputType'. Can optional be given by webmaster. Additional, the 'type="number"' will be automatically set, if the column is of type 'int' or if 'min' and 'max' is numerically.
